A small sleep-medicine series describes amnestic nocturnal scratching and responses to several treatments, including clonazepam in some cases.
What the source reports
The article combines four newly described patients with three previously published cases. Clinical histories and sleep studies supported discussion of an NREM arousal disorder. Some patients improved with clonazepam, while other interventions and comorbid conditions also influenced the clinical picture.
Limits and uncertainty
Seven reported cases are not seven newly recruited independent treatment comparisons. There was no randomized control group, and the authors’ proposed mechanism remains interpretive. Improvement with a drug does not establish that every scratching episode has the same cause.
Implications for forensic review
For a benzodiazepine-related behavior assessment, distinguish sleep behavior, awareness, memory, skin disease, medication effects, and other sleep disorders. The report supports a differential diagnosis. It cannot establish a general response rate or prove that clonazepam would resolve unexplained nocturnal injury in another patient.
