A before-and-after medication signal differs from evidence of a harmful drug–drug interaction.
What the publication shows
The register study screened fifty-three CNS medicines initiated during SSRI treatment using a Bayesian model. Alprazolam and flunitrazepam showed higher suicidal-behavior rates after initiation than before it. Comparisons outside SSRI treatment did not establish a harmful interaction specific to the SSRI combinations.
Limits of the evidence
The records could not distinguish augmentation, switching and treatment of comorbid illness. Indications, symptom severity and other concurrent medicines were incompletely captured. Worsening illness may influence both prescribing and the outcome.
Application to expert review
For causation analysis, separate the temporal signal, confounding by indication and the interaction question. Document symptoms before the new prescription. The paper does not establish that an SSRI–benzodiazepine combination caused an individual act or that the absence of an interaction signal means no clinical risk.
