A high-response dose estimate does not guarantee adequate or complication-free sedation.
What the paper found
Forty-eight children, with a median age of about twenty months, entered the remimazolam study. The estimated moderate-sedation ED90 was 0.28 mg/kg, with a confidence interval of 0.24–0.42. Respiratory depression and hiccups were among reported drug-related events; brain-network measurements were also explored.
Limits of the evidence
The authors acknowledged limited precision and neurological-population specificity. Imaging connectivity is not a direct measure of improved cognition, and a sedation response endpoint is not comprehensive safety.
Implications for case review
Keep the confidence interval and intended depth with the research estimate. It is not an individual dosing instruction or toxic threshold. Clinical interpretation requires the child’s neurological condition, other drugs, actual response and respiratory observations rather than assuming the modeled ED90 predicts every domain of effect.
