The parent drug and its active metabolite can change in different directions during an interaction.
What the supplied paper shows
A pharmacokinetic review discussed clobazam metabolism and interactions with cannabidiol and other antiseizure medicines. It described reports of increased N-desmethylclobazam exposure with CBD, sometimes without a corresponding parent-drug increase.
Limits of interpretation
The review combined different studies, regimens and populations. Its reported ranges cannot predict one patient’s concentration or establish a universal behavioral threshold. The underlying observations must be matched to the specific combination.
Implications for case review
A toxicology opinion should identify whether the assay measured clobazam, its metabolite or both. A stable parent result does not exclude altered total active exposure. Reconstruct dose changes, collection time, other enzyme modifiers and clinical observations. Distinguish a plausible metabolic mechanism from proof that the interaction caused an individual episode of sedation or loss of function.
