A composite exposure definition can include evidence with different meanings.
Study findings
The assessment linked prescription-monitoring, death-certificate and violent-death records for 3,465 Colorado suicide deaths in 2015–2017. Recent exposure meant a prescription within thirty days of death or a positive postmortem toxicology result. Twenty percent met that definition; nearly half of that group also had recent opioid exposure.
What limits the conclusion
A recent prescription does not confirm ingestion at death, while a positive toxicology result does not by itself establish a causal role. The study described exposure among deaths rather than comparing otherwise similar exposed and unexposed living populations. It therefore cannot estimate the causal effect of benzodiazepines on suicide risk.
Implications for case review
Identify which component establishes exposure in an individual case. Do not describe all qualifying cases as toxicology-confirmed. Clinical history, circumstances, complete toxicology and cause-of-death assessment remain necessary; surveillance co-occurrence cannot determine why a particular death occurred.
