Route and pretreatment duration define what an enzyme-probe result can show.
What the publication shows
Rats received Digeda-4 decoction repeatedly for fourteen days before an intravenous cocktail of probe medicines. Midazolam exposure and half-life increased while clearance decreased, supporting an inhibitory effect on the tested metabolic pathway under those conditions. Other probes showed different patterns.
Limits of the evidence
The experiment used rats, a defined preparation and a high experimental exposure. Species-specific enzyme biology limits translation. Intravenous probe administration does not directly test an herb’s effect on intestinal absorption or oral bioavailability, and the study did not measure human sedation.
Application to expert review
For an interaction opinion, identify the herb’s composition, schedule and actual route of the benzodiazepine. Do not transfer the observed magnitude to every traditional preparation or infer a patient-specific concentration change. The study provides a preclinical interaction signal rather than a validated clinical dose adjustment.
