Head position, stimulus response and muscle relaxation are different measures.
What the paper found
The horse study assessed detomidine–midazolam combinations using head height, acoustic response and tongue tone. All protocols reduced head height without a clear between-protocol difference, while other measures showed effects in the overall analysis.
Limits of the evidence
These veterinary pharmacodynamic observations cannot be transferred into human dose or impairment estimates. An overall statistical result does not establish that every pair of protocols differed. Combined agents also prevent assigning all relaxation to midazolam.
Implications for case review
Name the particular endpoint when describing an effect. A lowered head is not a complete measure of anesthesia, analgesia or safety. In veterinary case reconstruction, actual responses, administration and monitoring are more informative than treating every behavioral sign as an interchangeable sedation score.
