ERCP Meta-Analysis Found More Movement Alongside Less Hypoxia

A favorable safety profile can contain meaningful trade-offs.

Study findings

Five randomized trials involving 965 participants were pooled. Remimazolam was associated with less hypoxia, hypotension and bradycardia than propofol, but more tachycardia and body movement. Completion rates and selected delirium/agitation outcomes did not clearly differ.

What limits the conclusion

Only a few trials were available, and certainty varied by endpoint. Shared analgesics, procedural complexity and event definitions affect applicability. A pooled relative risk does not determine a particular patient’s absolute risk.

Implications for case review

Report both directions of effect rather than one global superiority label. Movement does not by itself prove awareness, and a lower hypoxia rate does not exclude respiratory depression. Review the actual monitoring, airway care and medication sequence when interpreting a specific ERCP event.
