Nonpharmacological support can be tested as an adjunct within a shared medication strategy.
Study findings
The trial compared midazolam alone with midazolam plus a stress ball or hand-holding and conversation. Anxiety ratings were lowest in the conversation/hand-holding group, which also had selected differences in anesthetic requirement and satisfaction.
What limits the conclusion
The combined intervention includes several interpersonal elements and is difficult to blind completely. Shared midazolam prevents interpreting the result as proof that support can always replace medication. Rating and anesthetic-consumption endpoints address different questions.
Implications for case review
Describe the common treatment and the added intervention. A calmer preoperative presentation does not establish absence of amnesia or impairment. For case review, consider communication, baseline anxiety, actual exposure and subsequent assessment rather than assigning every observed improvement solely to a drug or a social interaction.
