Lithotripsy Inflammatory Markers and Early Responsiveness Gave Different Recovery Signals

Lower biomarkers do not guarantee greater alertness immediately after surgery.

What the paper found

Eighty patients were randomized to remimazolam or propofol with other anesthetic medicines. Changes in IL-6, TNF-alpha and CRP favored remimazolam, while procalcitonin changes did not clearly differ. Despite shorter extubation timing, early MOAA/S and RASS scores were lower with remimazolam.

Limits of the evidence

Biomarkers are surrogate outcomes, and the study did not establish long-term organ protection. Extubation, responsiveness and inflammatory change are different measures. Shared adjuncts and the selected procedure limit broad attribution.

Implications for case review

Describe both favorable and less favorable early outcomes rather than substituting one global recovery label. An individual can meet airway criteria while remaining less responsive. Forensic review needs actual examinations, drug timing and complications, not an inference that lower inflammation proves complete cognitive recovery.
