Post-Endoscopy Lemborexant Series Had No Benzodiazepine-Free Sedation Comparator

An uncontrolled series cannot establish that a later sleep medicine prevented delirium.

What the publication shows

The retrospective pilot involved sixty-four pancreato-biliary patients with insomnia after endoscopy. All had received midazolam and pentazocine during the procedure. Sleep was reported in sixty-one patients; no overnight falls were observed, and one patient had a mild change in consciousness.

Limits of the evidence

The sample was small, selected and lacked an untreated or alternative-treatment control. Low event counts cannot exclude clinically important risk. Because procedural medicines were shared, the series cannot determine the independent delirium effect of midazolam or the protective effect of lemborexant.

Application to expert review

For chart review, separate procedural sedation, postprocedure sleep treatment and structured delirium assessment. Record the timing and character of altered consciousness. Absence of a documented fall or delirium episode is not equivalent to demonstrated prevention.
