A stakeholder review suggests that persistent prescribing can reflect shared expectations and system limitations rather than one clinician’s knowledge alone.
What the source reports
The authors synthesized views of healthcare professionals, older people, and family caregivers. Long-term psychotropic treatment was often accepted. Clinicians reported awareness of prescribing concerns but described barriers at individual, team, and organizational levels. Non-drug options were sometimes unavailable or unacceptable.
Limits and uncertainty
The synthesis concerns attitudes, not a controlled comparison of deprescribing strategies. Different psychotropic classes and care settings were combined. Language restrictions and the available interview literature may limit which perspectives were represented.
Implications for forensic review
For retrospective review, examine the patient’s priorities, prior attempts, access to alternatives, and follow-up arrangements. A general statement that benzodiazepines should be reviewed does not explain why treatment continued in one case. This paper offers reasons to investigate without proving that continuation was appropriate or inappropriate.
